What is Adult’s Continuing Care?

Adults continuing care is a package of support to meet both health and care needs. Provided by the (NHS) for a relatively small number of people, whose needs can't be met in the usual way through GP, hospital or community services.

If you're 18 or older and an assessment shows that your main care needs are health-related, you may qualify for NHS Continuing Healthcare.

It doesn't matter what specific illness or condition you have, who is providing your care, or where it's being given.


To figure out if you qualify, they'll look at four main things:
  • Nature: This is about what your needs are like and how they affect you, including what kind of help you need.
  • Intensity: This looks at how severe your needs are and how much support is necessary, especially if you need ongoing care.
  • Complexity: This considers how your needs show up and interact with each other, and the level of skill needed to manage them.
  • Unpredictability: This checks how much your needs change, and if it's tough to manage them without the right care, including the risks to your health if you don't get the help you need on time.


How do I ask for it?


A referral can be made by any health professional such as your GP, community nurse or another health professional who knows you, it can also be made by a social care professional, such as a social worker if you have one.

The full assessment process for NHS continuing healthcare usually involves 2 steps:
• Screening using the checklist tool
• A full assessment of eligibility using the Decision Support Tool (DST)

What help could I get?

The type of services and support will be different for every person, based on your health and care needs.


You can receive Adults continuing care in any setting (apart from acute hospitals) – including in your own home.

If you are eligible for NHS continuing healthcare in your own home, the NHS will pay for your package of care and support to meet your health and social care needs.


Continuing Care – Fast Track

If you have a rapidly deteriorating condition that may be entering a terminal phase, you may be eligible to receive urgent access to NHS continuing healthcare via fast track.


In the fast-track pathway, there is no requirement to complete a checklist or the decision support tool. Instead, an appropriate clinician will complete the fast-track pathway tool to establish your eligibility for NHS continuing healthcare.
